Insurance and Billing
As an out of network provider, please provide us with your insurance information before your appointment. We will be more than happy to contact your insurance company and get information regarding your specific plan. For major dental cases, as a courtesy, we can submit a predetermination so you will know your estimated portion ahead of time.
