Before your first visit:
Insurance information: As an out of network provider, please provide us with your insurance information before your appointment. We will be more than happy to contact your insurance company and get information regarding your specific plan.
Radiographs: If your dentist or physician has taken x-rays, please have them e-mailed or pick them up and bring them to our office prior to your scheduled appointment. Even though you may have recent routine dental films, we frequently need to take a special x-ray called a panoramic x-ray or panorex to more carefully evaluate your teeth and the supporting jawbone. If additional films are needed, we will take them at our office.
Complete health history forms: Please print and fill out or fill out online prior to your appointment.
What to bring to your visit:

- List of any medications
- Radiographs (if not emailed or mailed to us)
